Transcript Request Information
Transcript requests in the 15th Judicial District are governed under Administrative Order 24-5. Below are the standardized forms for the District.
Self Represented Litigants and Public
If you are representing yourself or are a member of the public making a request, please fill out Request for Transcript of Court Record AND Order for Transcript of Court Record. Requests and Orders shall be filed separately.
Request for Transcript Form
Please fill out the Request for Transcript of Record form and file in the corresponding case. Order for Transcript form must also be filed.
Order for Transcript Form
An Order for Transcript of Court Record must be filed separately but submitted with the Request for Transcript of Record.
Poverty Affidavit
Self Represented Litigants may fill out a Poverty Affidavit. The Poverty Affidavit must be filed separately than the Transcript Request and Order for Transcript.

Estimating Transcript Cost
Most transcripts require prepayment of estimated cost before transcript preparation will begin. Find exceptions listed in Administrative Order 24-5.
Formula to estimate cost
The estimated cost is calculated as follows: length of electronic recording in minutes times .75 equals estimated number of pages. Estimated number of pages plus cover page and certificate page, times page rate of 3.50 equals estimated cost of transcript. Each transcript should have a cover page and a certificate page.
Refund of Overpayment
Requests and Orders shall be filed separately.

To request to listen to an electronic recording within the 15th Judicial District, please fill out the form below and submit to the Clerk of the District Court in the county where the case is filed.
